This Liability Release is entered into between the parent or legal guardian and Galway Music Academy.

WHEREAS, the Parent desires to enrol their child in music lessons, classes, or activities provided by Galway Music Academy; and

WHEREAS, the Academy is willing to allow the Child to participate in said music lessons, classes, or activities under certain terms and conditions.

NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, the parties agree as follows:

1. Assumption of Risk: The Parent understands and acknowledges that participating in music lessons, classes, or activities may involve certain risks, including, but not limited to, physical injuries, accidents, or damage to personal property. The Parent voluntarily assumes all risks associated with the Child's participation in such activities.

2. Release and Waiver of Liability: The Parent, on behalf of themselves and the Child, hereby releases and discharges Galway Music Academy, its employees, agents, representatives, instructors, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to the Child's participation in music lessons, classes, or activities, including but not limited to, personal injury, illness, or property damage, whether caused by negligence, breach of contract, or otherwise.

3. Indemnification: The Parent agrees to indemnify and hold harmless Galway Music Academy, its employees, agents, representatives, instructors, and volunteers from any and all losses, damages, costs, or expenses (including reasonable attorney's fees) incurred as a result of any claims or actions brought against the Academy arising from the Child's participation in music lessons, classes, or activities.

4. Medical Emergencies: In the event of a medical emergency, the Parent authorizes Galway Music Academy to seek emergency medical treatment for the Child. The Parent agrees to be solely responsible for any medical expenses incurred as a result of such treatment.

5. Photography and Videography: The Parent grants Galway Music Academy the irrevocable right and permission to photograph and/or record the Child during music lessons, classes, or activities. The Academy may use these photographs and/or recordings for promotional purposes without any further consent or compensation.

6. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which Galway Music Academy operates.

7. Severability: If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

8. Entire Agreement: This Agreement constitutes the entire agreement between the parties regarding the subject matter hereof and supersedes all prior discussions, negotiations, or agreements, whether oral or written.

 

Consent for Use of Images and Videos

I, hereby grant consent for Galway Music Academy to capture, use, and publish images and videos featuring my child, for advertising and promotional purposes. This includes, but is not limited to, the school's website, social media platforms, brochures, and other marketing materials.

I understand that my child's images and videos may be used by Galway Music Academy to showcase student activities, performances, and achievements. These materials may be viewed by the general public, including prospective students, parents, and staff.

I acknowledge that Galway Music Academy will make reasonable efforts to ensure the safety and privacy of my child. However, I understand that once the images and videos are published, they may be shared, downloaded, or used by others beyond the control of Galway Music Academy.

This consent is valid until revoked in writing by the undersigned or until the student is no longer enrolled at Galway Music Academy.
